Hours of Operation:
You will need to bring the following information:
-Monthly income information on everyone in household (award letters or pay check stubs)and the first two pages of last year's Federal Income Taxes. If unemployed, zero income form is available at the office.
-List of all medication, dosage instructions, and name of the doctor that writes the prescription. (You can go to pharmacy to get print out of all medications currently taking or bring in prescription bottles.)
-Health Insurance Information
The Clark County Prescription Assistance Program helps those without prescription coverage or within the Medicare donut hole take advantage of assistance programs offered by the drug manufacturers. These programs offer prescriptions medications for free or at reduced cost for qualifying individuals. Eligibility is based on income guidelines set by the drug manufacturers. We process your information to find the best available program. Once we have printed your forms they will need to be taken to your doctor to sign and then mailed.

Event: Upcoming DIABETES Classes

The purpose of the Clark County Diabetes Coalition is to provide diabetes awareness and prevention in our community. The next meeting of the Clark County Diabetes Coalition (CCDC) will be May 9, 2013 at the health department the time 10:45am to 11:45am. You may call Carol Hisle for additional information. We welcome all new participants. The coalition will be working on planning an event for National Diabetes Month in November and the Diabetes Support Group Holiday meeting/dinner.
Diabetes Basics Class

This is an abbreviated version of the Diabetes Self-Management Series. It is a good choice if you have just been diagnosed with diabetes and are waiting to get into a diabetes self management series class or if you have never been to class. Call the health department at 744-4482 and ask

Event: Winchester's Weigh Down

Winchesterís Weigh Down is your chance to get in shape and shed unwanted weight. This 8 week weight loss competition offers classes to help you reach your goal. Weighing in every week holds you accountable and keeps you motivated. Start dates are March 13th and March 20th. There are awesome prizes for winners in 2 categories; Male winners & Female winners! ONLY $10 to sign up!
Participants in the Weigh Down can receive a 8 week YMCA membership for $48, starting March 13th. Call the YMCA at 859-744-9622.
College Park Health and Wellness Center is offering a reduced rate for swimming, 10 visits for $35 to Weigh Down Participants.
Upon Initial Registration, each participant must pay $10.
Weigh in at the same location every week. You will need your weigh in card that you received upon registration each time you weigh in.

Sat. May 11th - Public Trail Hike 10AM
Lower Howards Creek. Meet at Hallís on the River parking lot. Wear sturdy shoes and bring water. The hike will be 2 to 4 miles in length. Call Clare Sipple to register 744-4888.
Winners Announced at:
Run/Walk for the Paw 5K* on May 19th, 2013 at Bluegrass Community and Technical College starting at 2pm. ALL WWD Participants will receive free entry to the walk/run and a T-Shirt. Donations are accepted, all proceeds of the run/walk benefit the Clark County Animal Shelter.

Event: Cooper Clayton Smoking Cessation
Cooper/Clayton is an effective smoking cessation program that:
Addresses all aspects of smoking. Uses nicotine gum, patches or lozenges to control cravings. Provides intensive weekly education and support group sessions for 1 hour, each week for 13 weeks. Classes are held at the health department and in the community. Be sure to ask when you call to register.

How Much Does The Program Cost?
Nothing! The cost of the class and help guide are paid for by Clark County Health Department.
Worried about the cost of nicotine replacement products (NRP) like the patch, lozenge, or gum? Donít be! Nicotine replacement products usually cost about the same as what youíre currently spending on cigarettes. It may even be less! If you can afford to continue to smoke; you can afford to buy NRP.
Plus, we pay for 1/2 your patches. Participants pay for the first four weeks of patches and we pay for the last six weeks. Thatís 60% off!

About the Developers
Thomas M. Cooper, D.D.S., was a heavy smoker for 36 years. He made unsuccessful repeated attempts to stop smoking until he developed this program.
Richard R. Clayton, Ph.D. is an
internationally known expert on drug addiction. He teamed up with Dr. Cooper to help thousands stop smoking using the Cooper/Clayton Method.
